development of established endocarditis and symptoms. This is particularly true in pure mitral stenosis where symptoms may not begin until 20 years after the acute attack. Mitral stenosis represents a less severe degree of valve damage than does severe mitral regurgitation or mixed lesions in which the subvalvar apparatus, the chordae and papillary muscles, are involved as well as the valve cusps. In pure mitral stenosis symptoms begin gradually with dyspnoea on effort, tiredness, bronchitis in the winter, and often haemoptysis. The course tends to be one of gradually increasing disability punctuated by various crises (Goodwin, 1967).
